I hereby give my consent for my child to participate in all activities prepared by Dynamic Karate Academy during the daily operation of the Academy. In exchange for the Academy accepting said child’s application, I assume all risks and dangers associated with the conduct of the activities, and release, absolve and hold harmless The Academy and all of its respective officers, agents and representatives from any and all liability for any injuries to said child arising out of travel to, participation in or return from the selected camp sessions.

In the event of injury to said child, I hereby waive any and all claims against The Academy, including all coaches and affiliates, all participants, sponsoring agencies, advertisers and, where applicable, the owners and lessors of the premises used to conduct the event. There is a risk of injury inherent in all sporting activities, including Karate Kata & Kumite. Some of these injuries include, but are not limited to, the risk of fractures, paralysis or death.

Medical Authorization and Release

As the parent and/or guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency that, in the opinion of the treating medical professional, requires immediate attention to avoid further endangerment of the minor’s life, physical disfigurement, physical impairment or any other undue pain, suffering or discomfort, in the event of delay.

Permission is hereby granted to the treating physician to perform any minor medical or surgical treatment, x-ray examination and vaccinations for the named athlete. In the event of an emergency arising from a serious illness, the need for major surgery or significant accidental injury, I understand that every effort will be made by the treating physician to contact me in the most appropriate manner.
